چکیده

Abstract Post-operative chyle leak is a rare but recognised complication after abdominal surgeries particularly those involving dissection adjacent to the retroperitoneal lymph nodes and/or lymphatic systems. There are limited cases of post-operative associated with adrenal surgery reported in literature no consensus on its management. Lymphatics gland drain into thoracic duct via regional or directly cisterna chyli, posterior aorta. If not adequately managed, can be hypovolemia, electrolyte imbalances, malnutrition and immunosuppression. These consequences attributed loss fluid, electrolytes, lipids, proteins, immunoglobulins. We hereby report case 79-year-old male who underwent an open left adrenalectomy, nephrectomy, splenectomy distal pancreatectomy for large, 20.8cm cavernous haemangioma. Chyle was confirmed day 7 based high levels triglycerides output. Intra-operatively there evidence damage. However, major viscera close trunk predisposed patient this complication. The managed conservatively aim decreasing production by dietary manipulation use somatostatin. Enteral nutrition continued strict low-fat diet regular dietician input. strategies contributed gradual resolution patient’s recovery.

Post-thyroidectomy hypocalcemia exacerbated by chyle leak

UNLABELLED Transient hypocalcemia after thyroidectomy is not uncommon and the risk increases with the extent of neck surgery. We report a case of severe and prolonged hypocalcemia after total thyroidectomy complicated by thoracic duct injury. Hypoparathyroidism and thoracic duct injury are potential complications following total thyroidectomy with extensive lymph node dissection. This case sugg...

Management of Chyle Leak after Head and Neck Surgery: Review of Current Treatment Strategies

Chyle leak formation is an uncommon but serious sequela of head and neck surgery when the thoracic duct is inadvertently injured, particularly with the resection of malignancy low in the neck. The thoracic duct is the primary structure that returns lymph and chyle from the entire left and right lower half of the body. Chyle extravasation can result in delayed wound healing, dehydration, malnutr...

Etilefrine use in the management of post-operative chyle leaks in thoracic surgery.

Etilefrine, a sympathomimetic drug, was used 11 times in 10 patients with thoracic (n=8) or abdominal (n=2) chyle leak occurring after thoracic surgical procedures. It was given as a 4.2-5 mg/h intravenous infusion. During the 11 etilefrine administrations, three patients had total parenteral nutrition, three had enteral nutrition, three had oral fat-free diet and medium-chain triglyceride supp...

Management of Intra-operative Chyle Leak during Neck Dissection: Recognition and Control

Chyle fi stula is a rare but serious complication following neck dissection with an incidence ranging from 1% to 2.5% [1]. This uncontrolled extravasation of chyle arise from damage to the thoracic duct, which transports triglycerides and cholesterol from intestinal lymphatics into the venous system [2]. Beginning at the cisterna chyli, the thoracic duct ascends though the aortic hiatus of the ...
